On the 12:30 ET showing of WOF on GSN, something happened that I had not seen before. All three contestants missed the first toss up puzzle, which was "getting warmed up." Everything except the M was showing. Pat went on to the interviews. Then someone solved the next toss up, "Quiche Lorraine." I wondered what would happen in this case if they would have another puzzle. Since the first toss up doesn't determine who starts, they don't need to do another puzzle.

Play a new toss-up puzzle, edit out the unsolved one, throw up a disclaimer at the end of the show about a new puzzle being used. Done.

My understanding is that a dead-ball tossup returns the money to the prize budget and no replacement is deployed.

But the second toss-up is also for control.

I think in that case, they just automatically give the first spin of Round 1 to whoever is at the red arrow (like they did before toss-ups existed). (And in the case of no one getting the one before Round 4, control goes to whoever started Round 1).

And there's at least one example on the Tubes of a miss on the $2,000 puzzle:
